The first step in ordering from Elite is to fill out a credit application form and a credit card authorization form. You can download a .pdf version of each of these forms below.

After filling out the application, send it to Elite Orthotics, Inc. either by fax or mail for processing. Payment will be due upon receipt of brace (COD or Visa) until credit is established

There are three types of brace work orders. One for plastic braces, one for metal braces, and one for hybrid braces. You can download .pdf versions of these workorders below.

Simply fill out the work order form for the type of brace you need, and send it with the cast or tracing, or you can fax it separatly. The fax number and address are listed below.
